Your proposal form is a legal document. Fill it in like one.

Built for a real piece of work

Preparedness material for one licensed organisation. Adapt the working files before adoption; this is not legal advice.

The real risk in cyber insurance is not the premium. It is discovering mid-incident that your claim fails because an answer on the proposal form was wrong. Under the Insurance Act 2015 every answer is a legal representation, and the remedies for a wrong one run from proportionate cuts to your payout to the policy being treated as if it never existed, with premiums kept. In one widely reported case, an insurer walked away entirely over a misdescribed MFA deployment.

This pack turns the proposal form from a risk into a process. The guide explains the duty of fair presentation in plain English with the Act quoted, then works through the twelve controls underwriters ask about, what counts as evidence for each, and how to declare a partial control honestly so it becomes an underwriting conversation instead of a future dispute.

The workbook enforces the one rule that matters: never declare what you cannot prove. The attestation flags any control marked in place without provable evidence. The answer log records every submitted answer with its basis, who gave it and who approved it, which is precisely the record the law expects you to be able to show. A brandable copy is included, so the renewal pack you assemble for your broker is presented under your name. Renews in value every year at renewal time.
